I think it's manufacturers that update their systems. Example, Nvidia's Shield Android TV is not updated by google, but by Nvidia who updates the box themselves and usually add in even more additional features outside OS updates.
Some of each. Google updates Google's stuff and Sony update the core functions that allow AndroidTV to run on their hardware. Sony seem to be continuing to update their 2015 TVs and promise to update to Marshmallow.
I would think the manufacturers will need to provide the updates since there is light skin on their Android TV OSes (much like how Android on phones works). That's kind of why I went with a regular TV with a Nexus Player...I can swap those out when they become outdated.
