I live outside of the US (Cayman islands) and I would like to give up my cable tv, but still have the ability to watch cable. Someone mentioned using IPTV, can I get this on any android box or fire stick and do I need a vpn? Is there any other option I should explore?

It's two parts you need a provider and an app, some providers provide the app but not the best ones. Don't think we can discuss this much in this sub though.

Iptv is a perfect solution, as mentioned above you need to find a good provider and tivimate is the highest rated app. You shouldn't need a VPN of you have a good provider but sometimes it is necessary.
